Wellness Gift Card Policy

Wellness gift cards purchased through MOBI Physical Therapy & Wellness are intended exclusively for wellness-based services. Wellness services are non-medical in nature and are not intended to diagnose, treat, or manage medical conditions. These services do not constitute skilled physical therapy or medical care and do not replace evaluation or treatment by a licensed medical provider.

Each wellness visit is $99 per session.

Recipients of a wellness gift card must contact MOBI Physical Therapy & Wellness directly to schedule their wellness session(s). Appointments are subject to availability and must be scheduled in advance.

Wellness gift cards are transferable and may be used by another individual if the original recipient chooses to transfer the gift. However, gift cards are non-refundable, may not be redeemed for cash or credit, and have no cash value.

Gift cards may only be applied toward eligible wellness services as determined by MOBI Physical Therapy & Wellness and may not be applied toward physical therapy evaluations or treatment services unless explicitly stated in writing.

By purchasing or using a wellness gift card, the purchaser and recipient acknowledge and agree to the terms of this policy.
